Unfortunately it came back about a month later, but same process repaired it again. Why not have an app where you can simply see each \device id that corresponds to each bloody disk!!!!!! Disk 1 -> \device\harddisk#\DR# or something? In this case, the error is likely associated with a USB drive attached to the server. - Thus it was concluded that controller was faulty, and I went shopping for a new motherboard.

I have HP RAID management software but it didn't really help either since I have so many other hard drives (USB, iSCSI) that are not on the internal RAID.

I think maybe this post can help explain how to map between the number and the Hard-disk (look at photos especially), but if you have only one (most common), then there Could this be an issue with the enclosures and not the drives maybe a problem with the port multiplier and that is why it is only 3 out of the four Boot-up returned to normal speed and errors went away after disconnecting the card reader.
Find each DeviceType:REG_SZ:DiskPeripheral entry under the Targetid#\Logicalunitid0.
